Located in the heart of Sequatchie Valley and extending our services to neighboring regions, Achieve Success offers comprehensive ADHD evaluations.
To begin your ADHD evaluation with Achieve Success, ensure that you first discuss your concerns with your healthcare provider. Our appointment system operates exclusively through provider referrals. Once a referral is received, our office will reach out to schedule an evaluation appointment. Appointments are available on Fridays at our clinic. Upon completion, we fax evaluation results to your referring provider and also mail a copy directly to you.
Please note, while your referring provider can initiate treatment plans based on our evaluations, we do not prescribe medications.

Silke Hoosier
Nurse Practitioner (FNP-BC)
Born and raised in Germany, Silke Hoosier transitioned to the U.S. in her early adulthood and has been a proud resident of the Sequatchie Valley area since 2010.
Silke Hoosier began her healthcare career with pediatric nursing in Germany in 1997. She earned her BSN at Austin Peay State University in 2006 and later achieved her MSN from Middle Tennessee State University in 2017.
In her current role as a Family Nurse Practitioner (FNP) at Sequatchie Valley Primary Care, Silke goes above and beyond her duties. As a healthcare provider, she embodies the roles of listener and advocate.
She observed the challenges many patients faced in obtaining timely ADHD evaluations. This struggle often impacted patients’ academic progress, job performance, and personal relationships.
Recognizing these challenges, Silke pursued specialized training in ADHD evaluations, enabling her to personally conduct these assessments. Using tools such as the Brown ADD Scale, Conners Continuous Performance (CP-3) Assessment, Barkley’s Adult ADHD Checklist, and clinical interviews, she has taken the initiative to make these crucial services more accessible to the local community.
With a steadfast commitment to patient care and a fervent drive to enhance ADHD evaluation access, Silke Hoosier remains a symbol of dedication and expertise for all those she serves.
Outside of her professional commitments, Silke cherishes her role as a mother of three, and together with her husband, frequently finds joy and solace in nature, especially during camping trips and outdoor escapades.
